Health & Wellbeing Survey
Dear parent/carer,
South Lanarkshire’s Health and Wellbeing Survey for Children and Young People (2025)
South Lanarkshire Council is planning to undertake a health and wellbeing survey for children and
young people in November and December 2025.
We are conducting this survey because we would like to know more about the health and wellbeing
of children and young people in South Lanarkshire. We hope that the survey will allow us to:
• Collect information about the health and wellbeing of children and young people across South
Lanarkshire.
• Ensures children’s voices and experiences are taken into account when planning future
changes.
• Give head teachers information to improve the health and wellbeing support they give to
children and young people.
• Make sure our services are meeting the needs of children and young people.
• We aim to do the survey again in a few years’ time to look at whether children and young
people’s health and wellbeing is improving.
We are asking children and young people from P5 to S6 to take part in the survey.
We would like as many children and young people to complete the survey as possible. This will
help us to make sure their voices are heard.
The survey has been carefully and sensitively constructed following consultation with a range of
partners, including young people themselves and parents.
We have included the following information about the survey:

There is also further information, in order to help you decide whether to agree if your child takes
part in the survey. As the pupil information states, they will also be asked for their agreement to
take part.
Our website has further information, including all survey questions for different ages groups.
We hope you agree that surveys of this type are useful and will be in agreement to your child
taking part. We are hoping for a good response to the survey to ensure the findings are as
meaningful as possible and can help inform our decision making in the future.
If you have signed up to ParentsPortal, please answer the agreement question within the portal. If
you have not signed up for ParentsPortal, the school will send you information and an opt-in form
to agree to your child taking part. Please return the opt-in form to your child’s school office by 24
October 2025.
